The online form allows registration for up to 2 adults and 6 children. Please contact us at 773-838-8005 if you have more household members.
The following are requirements to complete the form: photo id, proof of income, medical card, or birth certificate.
If there you are the legal guardian, provide proof of guardianship, in case of adoption - provide a legal letter from the parent.
Proof of residency can be in the form of a bank statement,
cable bill, insurance, utility bill (gas or light), or recent mail with the last 3 months.

Online applications for Christmas Toys begin Monday, October 5th, and end Friday, December 4th.
There will be no "shopping" this year. This means that you will be given a pickup date and time to retrieve your gifts chosen by a volunteer
(based upon your suggestions) in a curb-side manner.
Gifts will be picked up at: The Salvation Army Midway Citadel, 5036-52 W. 47th Street, Chicago, IL 60638.
