The Collaborative Health & Wellness Service (CHWS for short) is based on patient-centered collaboration which includes a triad model comprised of primary care providers, case management, and a psychiatric system for individualized care. Since 2016, The Salvation Army of La Crosse County (TSA) has been cultivating a collaboration with local hospitals, the county, and the universities to meet the emerging need to provide a more effective and efficient way to for individuals/families to receive physical health, mental health, and preventative services.
CHWS partners include: The Salvation Army, Viterbo Master's in Mental Health Counseling Program, CARING nurses La Crosse County program, Viterbo counseling interns, and health care residents from Mayo Health System and Gundersen Health family practice and internal medicine department.
