Permanent Supportive Housing

When a family is in crisis, sometimes what's needed most is a place to call home. The Salvation Army's Permanent Supportive Housing program utilizes strengths-based case management, trauma-informed, and harm reduction models of care.  The program purpose is to provide permanent housing and case management to chronically homeless families and individuals, who have disabilities.  We assist participants to access community and mainstream resources, promote goal development and attainment that will assist participants to maintain their housing, increase their income either through entitlements and/or employment and to reach a level of stabilization and self-sufficiency that will allow them to move on to other types of funded permanent housing, if able.

  

SERVICES PROVIDED: Permanent housing, strengths-based case management, advocacy, recovery support, transportation assistance, household and hygiene assistance, food assistance, medication assistance, life skills training, landlord/tenant mediation, trauma informed care services, chaplaincy services and referral to legal assistance, job training and health care services. 

HOW TO ACCESS SERVICES: The Permanent Supportive Housing program utilizes our community’s Coordinated Entry system to when either program has open units.  People who are homeless can call the United Way 211 to set up a homeless assessment.  Assistance will be based on vulnerability and severity of service needs to ensure that people with the highest level of need receive housing in a timely manner.
