transitional housing ambassador program 

The Transitional Housing Ambassador Program provides affordable housing to adult men and women ages 18+ who are in need of temporary housing. The program offers two types of living quarters semi-private and dorm style rooms with different rates available. The Transitional Housing Ambassador Program is a FIVE-PHASE program-process of change and development.  Each phase is designed to help clients accomplish their goals and improve their overall success through structure, accountability, and tools for success. The goal of this program is to provide individuals with an opportunity to have a successful reintegration into the community by obtaining employment, private residence and/or community living. Each resident receives three meals, snacks, clean linens, access to an on-site computer lab and 12-step meetings.

The Center provides, for an additional fee, on-site laundry services, substance abuse treatment, medical screening and case management. Case Management and referral services are provided to assist with training, employment and housing.  The length of stay is between 30 to 180 days.

 

Admission Criteria

  • An assessment is required to enter The Transitional Housing Ambassador Program
    • An Assessment fee will be based on a sliding-fee scale

  • Individuals must pass a Covid-19 screening
  • A photo ID is required (original or paper copy) at the time of admission
  • Individuals must be able to perform ADLs (Activities of Daily Living) independently
  • For the safety of all clients, no medical diagnosis of Dementia or Alzheimer’s is admitted into Transitional Housing/Ambassador
  • For the safety of all clients, individuals cannot be admitted with IV’S, Pic Lines, or Catheters
  • Individuals must be mobile (if in a wheelchair, must be able to push and safely transfer themselves)
  • A medication list and all medications must be brought at the time of admission
  • A medical release form must be provided for:
    • All current medications
    • A Mental Health or Medical Diagnosis
    • Leaving the hospital

What to Expect

  • Individuals must be willing to work a five-phase program. The phases are designed to introduce a program of recovery and add accountability and structure to the program. With each phase, the individual will earn a later curfew and more privileges including overnight passes.
  • Individualized treatment plans will be developed with a counselor that identifies treatment goals and objectives. Regular follow-ups will occur through counseling services.
  • Individual and group counseling that utilize person-centered therapeutic approaches including cognitive behavioral therapy and motivational interviewing.
  • Daily access to case management services.
  • Psychoeducational groups and life skills groups including time management, finances, and budgeting.
  • In-house 12-step meetings (Narcotics Anonymous, Cocaine Anonymous, Alcoholics Anonymous, Heroin Anonymous, and Celebrate Recovery)
  • Church and spiritual services.
  • Outpatient and Aftercare Services
  • Employment services—Assistance with resume building, job searching, interviewing skills training, and job placement.
  • Access to medical care and visits with a nurse practitioner.
  • Assistance signing up for insurance.
  • Curfews
  • Security staff on-site 24 hours a day.
  • Random drug screens and breathalyzer tests.
  • All meals will be provided including breakfast, lunch, and dinner. Sack lunches are provided to individuals who work, go to school, or have appointments.
  • Dorm-style living with semi-private bedrooms.
  • To always be respected, supported, and empowered.

What to Bring

  • Clothing
    • No tank tops
    • No clothing with inappropriate messages, drugs, alcohol, tobacco products, or gang affiliation
    • If leggings are worn you must have a long top to cover the buttocks
    • Shorts, dresses, and skirts must come below the knee
    • Holes or rips are not permitted in jeans/pants
    • Hygiene products
    • Shampoo, conditioner, shower shoes, soap, razors, etc.
  • Laundry detergent/change for the laundry machine if you wish to do your laundry in-house
  • Cell phone
  • Alarm clock
  • A lock for the bedroom lockers
    • There are lockers inside each bedroom to store belongings. If a client wishes to place a lock on their locker, they must supply their own lock. An additional key and/or combination must be provided to Transitional staff in the event of an emergency.
  • Totes or other bedroom organizational supplies
  • Medication will be kept in and accessed through the medication room
  • $5 for a tobacco locker if the client smokes
  • No outside food or drinks

It is recommended to avoid bringing valuable property as HLC is not responsible for the loss, damage, or destruction of any personal property.
