ROOTS: Supportive Permanent Housing

The ROOTS Permanent Housing program will provide stable housing and case management services to assist chronically homeless individuals with a disability.

  • Residential: Located at 5433 W. National Ave. West Milwaukee, WI 53214, participants are housed in a residential community.
  • Occupancy: 15 partially furnished apartments are available to house chronically homeless men and women.
  • Opportunity: The participant has the opportunity to be self-sufficient and productive in their community.
  • Towards: With guidance and support, program participants work towards self-sufficiency and becoming productive members of the community.
  • Stability: Stable housing enables the participant the opportunity to establish roots, experience personal growth and sustain self-sufficiency.

Eligibility Criteria

  • Individual men and women
  • 18 years or older
  • Chronically homeless with a disability: Chronically Homeless is defined by having at least one continuous year of homelessness or having 4 episodes of homelessness in the past 3 years
  • Must pay 30% of their income in rent
  • Income is not necessary upon admission to the program, but obtaining income to meet living expenses will become part of each program participant’s case plan
