  2. Bring along the documents listed below to your appointment.

Documents Needed for Applications: 

  • Photo I.D. (required)
     
  • Proof of Residence: (One listed item is required)
    Photo I.D. with current address
    Rent lease or agreement
    Piece of mail with current postmark
    Utility Bill
     
  • Proof of Income: (One listed item is required)
    Check stubs
    Copy of SSI or SSD check
    2023 Income Tax records
     
  • Establishment as a Member of the Household: (One listed item for each family member receiving assistance is required) 
    Birth Certificates
